ABSTRACT

Objective: To explore the long-term effect of combined surgery for the treatment of congenital tibial pseudarthrosis in children. Methods: The clinical data of 44 children with congenital tibial pseudarthrosis who underwent combined surgery (tibial pseudarthrosis tissue resection, intramedullary rod fixation, wrapped autologous iliac bone graft, Ilizarov external fixator fixation) from August 2007 to October 2011 at the Department of Pediatric Orthopedics, Hunan Children's Hospital were collected. There were 33 males and 11 females. The age at the time of surgery was (3.7±2.2)years (range:0.6 to 12.4 years), including 25 cases under 3 years old and 19 cases above 3 years old.Among them, 37 cases were complicated with neurofibromatosis type 1.The operation status, postoperative complications and follow-up results were recorded. Results: The follow-up time after surgery was (10.9±0.7)years (range:10 to 11 years).Thirty-nine out of 44 patients (88.6%) achieved initial healing of tibial pseudarthrosis, with an average healing time of (4.3±1.1)months (range:3 to 10months).In the last follow-up, 36 cases (81.8%) had unequal tibial length, 20 cases (45.4%) had refractures, 18 cases (40.9%) had ankle valgus, 9 cases (20.4%) had proximal tibial valgus, and 11 cases (25.0%) had high arched feet.Nine cases (20.4%) developed distal tibial epiphyseal plate bridging.17 cases (38.6%) had abnormal tibial mechanical axis.Seven cases (15.9%) developed needle infection, and one case (2.3%) developed tibial osteomyelitis. 21 patients (47.7%) had excessive growth of the affected femur.Five patients (11.3%) had ankle stiffness, and 34 patients (77.2%) had intramedullary rod displacement that was not in the center of the tibial medullary cavity.Among them, 8 cases (18.1%) protruded the tibial bone cortex and underwent intramedullary rod removal.18 children have reached skeletal maturity, while 26 children have not been followed up until skeletal maturity. Conclusion: Combined surgery for the treatment of congenital pseudarthrosis of the tibia in children has a high initial healing rate, but complications such as unequal tibia length, refracture, and ankle valgus occur during long-term follow-up, requiring multiple surgical treatments.

ABSTRACT

Objective:To analyze the clinical characteristics of elderly patients with papillary thyroid cancer(PTC)by age grouping. Methods:The patients were divided into three groups according to age at diagnosis: old(≥60 years, 90 cases), middle(40-59 years, 359 cases)and young patients(<40 years, 203 cases). The clinical data of 652 patients with PTC who were admitted to the Department of Thyroid Surgery of Beijing Hospital from December 2019 to December 2021 were retrospectively analyzed. The patients were divided into elderly group(≥60 years old, 90 cases), middle-aged group(40-59 years old, 359 cases)and young group(<40 years old, 203 cases). The clinical characteristics, ultrasound characteristics and invasion-related factors of patients in different groups were analyzed by statistical methods. Results:Compared with the young and middle-aged group, the elderly patients with PTC had a higher proportion of microcarcinoma(75.6%, 75.2%, 61.4%, χ2=13.054, P=0.001), less cervical lymph node metastasis(24.4%, 34.3%, 58.1%, χ2=41.650, P<0.001), and lower proportion of metastatic lymph nodes(0.08, 0.14, 0.24, χ2=40.618, P<0.001). There was no significant difference in tumor location and extra glandular invasion among the three age groups(35.56%, 36.2%, 38.4%, χ2=0.959, P=0.545). Conclusions:Compared with the young and middle-aged groups, PTC showed low invasiveness in the elderly population. In addition to surgical treatment, for elderly patients with low-risk clinical characteristics, it is worth performing a further study on whether careful observation or palliative treatment can be selected after comprehensive evaluation.

ABSTRACT

Objective:To evaluate the effects of weight-bearing area compression injury of the femoral head on the prognosis of elderly acetabular fractures after open reduction and internal fixation. Methods:A retrospective analysis of 36 elderly patients with acetabular fractures treated with open reduction and internal fixation during January 2014 to January 2018 were conducted. All patients with compression injury of the weight-bearing area of the femoral head, including 22 males and 14 females with 73.2±6.5 years old (range 60-87 years old), were included. The compression injury of weight-bearing area of the femoral head was not treated. According to the Letournel-Judet classification of acetabular fractures, there were 14 cases with both-column, 12 cases with anterior column and posterior hemitransverse, 4 cases with T type, 4 cases with transverse, and 2 cases with posterior column+ posterior wall. A total of 14 cases were accompanied by acetabular joint surface compression, while 29 cases were accompanied by joint dislocation. The Merle d'Aubigné score was used to evaluate the hip function during follow-up. The Matta classification method was used to evaluate the results of acetabular fracture reduction. The Kellgren-Lawrence classification standard and Ficat-Alert staging method were used to evaluate the traumatic arthritis of the hip and femoral head necrosis, respectively. During the follow-up, the femoral head necrosis with stage III, IV, or traumatic arthritis III, IV, or with indications for joint replacement was defined as surgery failure. CT scans of the pelvis were performed before and at 2-5 days after operation. The compression size of the femoral head on the coronal and axial planes of the CT scan was calculated for the compression volume. The compression severity was divided into small (<1 cm 3), medium (1-2 cm 3) and large (>2 cm 3) according to the volume. Binary Logistic regression analysis was used to analyze whether the postoperative measurement of the femoral head compression volume was associated with the risk of surgical failure. Results:All patients were followed up for 34.7±8.9 months (range 25-54 months). There were 7 cases with large compression of femoral head, 14 cases with medium, and 15 cases with small pre-operatively. However, there were 12 cases, 10 cases and 14 cases with large, medium and small at 2-5 days after operation, respectively. Six cases were excellent reduction, 22 cases were good, and 8 cases were poor. Thus, the excellent and good rate was 78% (28/36). At the last follow-up, Merle d'Aubigné score was excellent in 2 cases, good in 8 cases, fair in 5 cases, and poor in 21 cases. The excellent and good rate was 28% (10/36). There were 20 cases with surgery failure with 56% (20/36) failure rate. There were no statistically significant differences in the patient's age, body mass index, operation duration, blood volume, fracture type, fracture reduction, combined acetabular joint surface compression, and combined joint dislocation between the two groups. However, there was a statistically significant difference in the frequency distribution of compression volume in the weight-bearing area of the femoral head after surgery (χ 2=22.047, P<0.001). In patients with large, medium, and small-volume compression of the femoral head weight-bearing area, the surgery failure rates were 92%, 80%, and 7%, respectively. The large and medium-volume compression of the femoral head weight-bearing area were independent risk factors for surgical failure. Conclusion:Open reduction and internal fixation can be used to treat elderly patients with acetabular fractures combined with femoral head compression injury. Despite satisfactory reduction for acetabular fractures, the larger volume of femoral head compression affects the clinical outcomes with extremely high rate of surgical failure within 2 years.

ABSTRACT

Objective: To evaluate the effect of nickel-titanium three-dimensional memory alloy mesh combined with autologous bone for living model of canine tibial plateau collapse fracture by biomechanical testing. Methods: Sixteen healthy 12-month-old Beagle dogs were randomly divided into 4 group, 4 dogs in each group. The dogs were used to establish the tibial plateau collapse fracture model in groups A, B, and C. Then, the nickel-titanium three-dimensional memory alloy mesh combined with autologous bone (the fibula cortical bone particles), the artificial bone (nano-hydroxyapatite), and autologous fibula cortical bone particles were implanted to repair the bone defects within 4 hours after modeling in groups A, B, and C, respectively; and the plate and screws were fixed outside the bone defects. The dogs were not treated in group D, as normal control. At 5 months after operation, all animals were sacrificed and the tibial specimens were harvested and observed visually. The destructive axial compression experiments were carried out by the biomechanical testing machine. The displacement and the maximum failure load were recorded and the axial stiffness was calculated. Results: All animals stayed alive after operation, and all incisions healed. After 1-3 days of operation, the animals could stand and move, and no obvious limb deformity was found. The articular surfaces of the tibial plateau specimens were completely smooth at 5 months after operation. No obvious articular surface collapse was observed. The displacement and maximum failure load of specimens in groups A and D were significantly higher than those in groups B and C ( P0.05). Conclusion: The nickel-titanium three-dimensional memory alloy mesh combined with autologous bone for subarticular bone defect of tibial plateau in dogs has good biomechanical properties at 5 months after operation, and has better axial stiffness when compared with the artificial bone and autologous bone graft.

ABSTRACT

Xanthine oxidase (XO) is a key enzyme in the synthesis of uric acid. Therefore, XO inhibitors play an important role in the antihyperuricemic therapy. Based on the template structures of febuxostat and topiroxostat, 18 amide derivatives were designed and synthesized. Among them, six showed apparent inhibitory activity against XO under the concentration of 10 μmol·L-1. Molecular docking revealed the possible interaction mode of this compound class, which may provide a clue for further molecular design.

In both manipulation groups, Cuanzhu (BL 2),Yuyao (EX-HN 4), Sizhukong (TE 23), Taiyang (EX-HN 5), Fengchi (GB 20), Zusanli (ST 36), Guangming (GB 37) and Sanyinjiao (SP 6) were punctured, but stimulated with rotating manipulation and lifting-thrusting manipulation respectively three times per week, 10 times as a treatment session and totally one session was required. In the corrective lenses group, the glasses were applied at daytime. The clinical efficacy and the changes in distant vision of naked eye before and after treatment were compared among the three groups.</p><p><b>RESULTS</b>The total effective rate was 87.5% (105/120) in the rotating manipulation group, which was better than 69.2% (83/120) in the lifting-thrusting manipulation group (P < 0.05). The distant vision of naked eye was improved apparently in the rotating manipulation group and the lifting-thrusting manipulation group after treatment (both P < 0.05). But it was not improved in the corrective lenses group (P > 0.05). The distant vision of naked eye was improved more apparently after treatment in the rotating manipulation group as compared with that in the lifting-thrusting manipulation group (0.75 +/- 0.23 vs 0.68 +/- 0.24, P < 0.05). For 96 cases (192 eyes) with acupuncture treatment, in 3-month follow-up, 87.0% (167/192) of the cases maintained the stable vision as the original level and 13.0% (25/192) of them were reduced in the vision In the acupuncture groups, it was found that the improvement of distant vision of naked eye was more obvious after treatment with younger age, better basic vision and shorter duration of sickness (all P < 0.05).</p><p><b>CONCLUSION</b>Acupuncture achieves the positive and sustainable clinical effect on juvenile myopia, and the results of rotating manipulation are superior to that of lifting-thrusting manipulation. Age, basic vision and duration of sickness impact the clinical efficacy.</p>

ABSTRACT

Objective To investigate the clinical curative effect of“Qing Dynasty Palace manipulation” in combination with plaster immobilization and foot bath with traditional Chinese medicine for acute soft tissue sprain of the ankle.Method A total of 90 patients with acute soft tissue sprain of the ankle from emergency department of WangJing Hospital, CACMS were recruited from July 2013 to April 2014 and randomly divided into two groups with 45 patients in each. The patients in the treatment group were treated with the court manipulation, plaster and Chinese herbs washing, and those in the control group were only treated with cast immobilization. The clinical curative effect was evaluated using Baird-Jackson score system(excellent: 96-100; good: 91-95; fair: 81-90; poor: 0-80).Result At the second, third week after treatment, the excellent/good rates in the treatment group were significantly higher than those in the control group(at week 2: 73.3%vs. 44.4%;χ2=11.468,P<0.01; at week 3: 86.7%vs.60.0%;χ2=6.875,P<0.05). At week 2 and 3, Baird-Jackson scores in the treatment group(87.41±14.12, 93.16±9.51)and in the control group(76.32±11.41, 82.54±9.22)were significantly higher than those before treatment(treatment group: 72.11±12.61; control group: 71.67±13.12, allP<0.05); at week 3, Baird-Jackson score in the treatment group were significantly higher than that in the control group(P<0.05).Conclusion“Qing Dynasty Palace manipulation” in combination with plaster immobilization and foot bath with traditional Chinese medicine can increase Baird-Jackson score and aid recovery of the ankle function in patients with acute soft tissue sprain of the ankle.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effect of different methods of physical therapy on plantar fasciitis.</p><p><b>METHODS</b>From June 2009 to March 2012,30 patients with plantar fasciitis were randomly divided into 3 groups including phonophoresis (PH) combined with stretching exercise, ultrasound (US) combined with stretching exercise, stretching exercise, 10 patiens in each group. In stretching exercise group, there were 2 males and 8 females with an average age of (46.7+/-6.5) years old,the mean constitutional index duration was (26.7+/-2.8) kg/m2. In US combined with stretching exercise group, there were 4 males and 6 females with an average age of (45.8+/-6.1) years old,the mean constitutional index duration was (26.4+/-3.4) kg/m2. In PH combined with stretching exercise group, there were 3 males and 7 females with an average age of (48.4+/-8.0) years old,the mean constitutional index duration was (25.4+/-3.0) kg/m2. Patients in PH and US were treated for 10 min everyday by ultrasound, 5 times per week, lasted for 4 weeks; and patients by ultrasound therapy in PH were treated with diclofenac diethylamine at the same time. All the 30 patients received instruction for stretching exercises at home. Pain and ability to function were evaluated before treatment, immediately afterwards, and three months later. Morning pain was evaluated by VAS, and the sub-scale of FFI evaluated the affected foot function.</p><p><b>RESULTS</b>Patients's general status and original pain state of plantar fasciitis before treatment had no significant difference among three groups. There were statistical differences of morning pain and FFI-disability score between PH group and stretching exercise group at 1 month (P<0.05), and no statistical differences among three groups at 3 months (P>0.05). Compared with before therapy, the pain and disability score of three groups significantly improved in the three points of time (P<0.05).</p><p><b>CONCLUSION</b>Stretching exercises and combining with PH or US are effective for pain and disability in patients with plantar fasciitis and that addition of PH to exercise therapy betters the effectiveness.</p>

ABSTRACT

Objective To investigate decision making under risk in patients with antisocial personality disorder(ASPD).Methods The game of dice task (GDT) was a commonly used measure of decision making under risk.31 ASPD patients and 33 healthy controls (HC) were investigated by GDT with explicit probability. Results ASPD patients performed poorly in the entire task.ASPD selected more risky options (ASPD:10.06 ±5.26,HC:5.42 ± 3.29,t =4.201,P < 0.01) compared with healthy controls. The most frequent choice made by ASPD patients was two numbers.Accordingly, the most frequent choice made by HC group was three numbers(one number:the median of ASPD patients was 1 (1-8),the median of HC was 1 (0-2.5),Z =-2.295,P =0.022) ;three numbers:ASPD:4.77 ±4.23;HC:6.79 ±3.43; t=-2.100,P=0.04;four numbers:ASPD:3.06 ±3.53;HC:5.82 ± 3.41 ; t =3.176,P=0.002).The frequency of choosing the risky options had correlation with the rate of using negative feedback(r =-0.613,P < 0.01),impulsiveness (r =0.481,P =0.006) and the results of Stroop test (r =0.566,P < 0.01).Conclusion Main results reveal that patients with ASPD make risky decisions significantly more often than the HC.Moreover, they show lack of capacity to advantageously utilize feedback, and correlation with executive function, impulsiveness, and negative feedback.

ABSTRACT

Objective To investigate the capabilities of decision-making in male subjects with Antisocial personality disorder (ASPD).Methods A total of 76 male ASPD subjects were compared with matched 60 healthy controls on the Iowa Gambling Task.At the same time,All of them were evaluated with Self-Rating Anxiety Scale (SAS),Self-Rating Depression Scale (SDS),Mini-Mental State Examination (MMSE),the MMPI Personality Disorder Scales (MMPI-Pd) and The Barratt Impulsiveness Scale (BIS-11).Results The ASPD group selected less advantageous cards (the number of cards selected from the advantageous decks minus the number of cards selected from disadvantageous decks) than healthy controls ((-4.13 ± 18.27) time,(6.03 ± 19.43) time respectively,t (134) =-3.132,P =0.002).A 2 (group) × 5 (block) ANOVA (analysis of variance) on the cards of net advantageous from the gambling task revealed a significant main effect of group (F(1,134) =9.811,P=0.002).Meanwhile,the ANOVA revealed a significant main effect of block (F(4,5336 =15.456,P < 0.001).The results showed that healthy controls and ASPD group gradually shifted their selections toward the good decks as the game progresses. The study indicated the total number of advantageous cards for subjects with antisocial personality disorder were negative correlation to the MMPI-Pd(r =-0.232,P=0.006).Conclusion The present study suggests that the decision-making impairment might be in the male subjects with antisocial personality disorder. The deficit of decision-making for ASPD might be attributed to the dysfunction of the orbitofrontal, dorsolateral frontal, and anterior cingulate cortex.
